Proven to reduce ED LOS and improve LBTC rates, the ADTA model evaluates patient data from the moment an ED encounter begins, providing a real-time, dynamic decision to admit (DTA) score that proactively guides the admission process.
Anticipated ED Discharge
(AEDD)
Our AEDD model serves as a tool to help hospitals reduce crowding in the emergency department and improve bed occupancy rates. A complement to our ADTA model, the AEDD model leverages real-time patient data to anticipate patient discharge. The model empowers care teams with the information they need to prepare for release and notify patients and caretakers of likely outcomes, improving patient flow efficiency and the patient experience overall.
How Choreo-ED Works
Following a SOC2 framework and HIPAA compliance, the Choreo-ED suite of solutions integrates directly into a hospital’s EHR system. As patient data, physician orders and lab results are recorded, the model utilizes real-time inputs and rich historical data to generate a dynamic anticipated admission or discharge score.
When the likelihood of admission or discharge exceeds a certain threshold, a notification is triggered, indicating to the care team that a patient will, with high confidence, be admitted or discharged soon. From the point of notification, care teams can begin to proactively initiate key tasks in the patient flow.
